Schizoaffective Disorder
Schizoaffective disorder combines a schizophrenia-like illness with major mood episodes, but the diagnosis depends on how the psychosis and mood symptoms relate over time.

Timing in context
There is no 6-month minimum like schizophrenia. Instead, the clinician must understand the illness across time and confirm both psychosis without mood symptoms and mood symptoms during most of the illness.
How to remember it
- 1Together: a major depressive or manic episode occurs during the same illness period as schizophrenia-level psychotic symptoms.
- 2Psychosis alone: delusions or hallucinations continue for at least 2 weeks when a major mood episode is not present.
- 3Mood dominates the course: major mood episodes are present for most of the active and residual illness.
Common confusion
If psychosis occurs only during mood episodes, consider a mood disorder with psychotic features instead. If mood episodes occupy only a minority of a chronic psychotic illness, schizophrenia may be the better fit.
